Chronic Disease Self-Management Program offered in Fort

    FORT FAIRFIELD — The Living Well Chronic Disease Self-Management Program is a six-week course that begins Oct. 8 at the Fort Fairfield Health Center.     This free program, offered by The Aroostook Medical Center in coordination with the Aroostook Agency on Aging, is for anyone living with a chronic disease like asthma, arthritis, cancer, COPD, heart failure, diabetes, depression or obesity. Participants will learn to cope with and manage health problems by:
• Setting doable goals;
• Working with others;
• Finding support and solutions to problems;
• Making daily tasks easier;
• Relaxing and managing stress; and
• Working in partnership with a healthcare team.
    After completing the course, participants will be better prepared to cope with sadness, deal with pain and tiredness, eat for better health, manage medications, talk with family, friends and healthcare providers, and relax and enjoy life.
    All classes will be held in the Conference Room at Fort Fairfield Health Center. Participants are encouraged to bring a family member, friend or caregiver. Pre-registration is required. For more information or to register, please call 768-4160.
